In consideration of my participation in the Martial Arts, Boxing, Kickboxing, and Fitness Training Classes/Programs, I, the undersigned student or parent/guardian, acknowledge that there are inherent risks involved in such activities. I hereby assume all risks associated with my participation in these classes/programs.
I further agree to release and hold harmless Liverpool Martial Arts Inc., Baldwinsville Martial Arts LLC, Champion FX LLC, Team Andrello, and their respective management, staff, agents, and fellow participants from any and all liability for personal injury, property damage, or loss of personal belongings that may occur during the course of training or as a result of my participation.
I affirm that the student(s) named above are physically fit to participate in the prescribed activities and do so voluntarily, in exchange for the agreed-upon fee. I also understand that all fees paid are non-refundable.
